We offer a variety of substance use and mental health assessments to understand your strengths, needs, and the right level of care. Assessments are used to tailor a personalized treatment plan.

Substance Use Assessments

Global Assessment of Individual Needs (GAIN): Measures scope of issues related to substance use disorder and utilization of services. Mandated by the state for all court-ordered clients. Approximately 3.5 hours to complete in the office.

Substance Use Assessment: For those not required to complete a GAIN, this 2 hour assessment will assess needs and appropriate level of care

Mental Health Assessment

Comprehensive Diagnostic Assessment (CDA): An in-depth evaluation of mental health and substance use concerns to determine the appropriate treatment approach.

Mental Health Assessments: A variety of screenings and assessments to assess emotional, behavioral, and psychological well-being.

Child & Adolescent Needs and Strengths (CANS)

A multi-purpose tool to assess children and adolescents for service planning and develop a tailored treatment plan. This assessment takes up to 2 hours to complete.
